  • FamilyID - Massapequa's Online Sports Clearance site. For your student to be cleared to play, you must complete FamilyID Form and provide a current physical. Either upload a current image of your completed physical form to the FamilyID sports registration site or bring a copy to the nurses' office.

    NYDPHSSA Section VIII - Gives athletes, coaches and parents access to Section VIII sports resources.

    NYS Physical Form -  NYSED requires an annual physical exam for interscholastic sports.

    Parent Handbook - Prepared as a reference for student-athletes and their parent/guardians in an effort to define responsibilities as they pertain to the rules and regulations, as well as, the policy and procedures for participation in the interscholastic athletics program.

    Physicals - All necessary forms must be properly completed and submitted using the online registration program FamilyID.

    Playoff Tickets - GOFAN online site to purchase tickets to upcoming events.
