Physicals

All participating students must...

1. Complete FamilyID Form:

  • Includes important health information reviewed by the Nurses office prior to each season as well as mandatory policies and agreements.

2. Provide a Current Physical:

  • All Athletes must have clearance prior to the 1st day of tryouts (NO EXCEPTIONS)

    Athletes will not be given a physical without submitting a signed Health Appraisal Form - obtainable form the school nurse/health office. If an athlete misses the Sport Physicals scheduled, they must schedule their own appointment with their own physician at their expense and the doctor must complete the NYS Physical Form. 

    NYS PHYSICAL FORM

    Athletes will not be given a physical without submitting a signed Health Appraisal Form. Forms are obtainable from the school nurse/health office.   

Winter Physicals Schedule

  • All Athletes must have clearance prior to the 1st day of tryouts (NO EXCEPTIONS)

    In-School Physicals for Winter 2019 Sports will be given on the following dates in the Nurse's Office:

    • Massapequa High School: Tuesday, November 12, 2019 starting at 8:30am
    • Ames High School: Thursday, November 14, 2019 starting at 8:30am
    • Berner Middle School: Thursday, November 7, 2019 starting at 9am
    • Berner Middle School Winter II: Thursday, January 16, 2020 starting at 9am

    Athletes will not be given a physical without submitting a signed Health Appraisal Form. Forms are obtainable from the school nurse/health office.   

SPRING PHYSICALS SCHEDULE

  • All Athletes must have clearance prior to the 1st day of tryouts (NO EXCEPTIONS)

    Physicals for Spring 2020 Sports will be given on the following dates in the Nurse's Office:

    • Massapequa High School: Tuesday, February 25, 2020 starting at 8:30am
    • Ames High School: Tuesday, March 3, 2020 starting at 9am

    Athletes will not be given a physical without submitting a signed Health Appraisal Form. Forms are obtainable from the school nurse/health office.   

Important Reminders:

    • Physicals expire 12 months after the date of health exam.
    • No athlete will be permitted to play or tryout with an expired physical.
    • Hard copy physical forms and health documents will not be accepted at any time
